Medical Conditions

Does your child have allergies, asthma, diabetes, seizures, CP, MS, CA, or any other chronic health condition?

Help us to help your child, by ensuring we have a Henry County Schools action plan from your child's doctor on file. Austin Road Middle School will provide parents with the form to take to the physician. 

Parents MUST return the form to the clinic. These forms also tell us how your doctor would like us to care for your child.

If your child has a chronic condition, we will happily work with you in following doctors orders.
Medications remain in the clinic at all times. 
 
We are always happy to work with your doctor and will provide information as you require.
