General medication/prescription dispensation to your child by ARMS staff

If a child needs to take over-the-counter or doctor-prescribed medication while at school, parents will need to read, complete, and sign a STUDENT HISTORY & MEDICATION AUTHORIZATION form.

Provide the medication and anything needed to dispense it (i.e., measuring spoon, nebulizers, aero chambers etc.).

Please remember that medications, whether over-the-counter or prescribed, should NOT be transported by students. Parents will need to deliver medication to the school.

Any and all medication must be in the original packaging, and the student's name, ID#, and grade should be clearly marked on the package or bottle.

Should your child require an inhaler, please have the pharmacist label the inhaler NOT the box, since boxes tend to get damaged. If the inhaler does not have a label, we cannot allow it to be used. Your pharmacist will print a duplicate label for this purpose at NO cost.

We cannot accept pills that are not in containers.

Samples MUST be prescribed by a doctor.
